If you have a computerised repeat prescription slip, please tick the drugs required and place it in the box provided at the Medical Centre or send by post (please enclose an S.A.E. if you wish the prescription to be returned by post). If you do not have a computerised slip please see the receptionist.

The local chemists all offer a collection/delivery service which you are welcome to take advantage of.

Please allow 2 working days before collection.
Telephone requests can pose a security risk and so these can only be accepted in extenuating circumstances. They can also cause confusion, block other calls and take up valuable staff time.
The doctor will need to review your medication annually; please consult in the month your review is due (stated on the slip).
